• A.  Complete the clinical observer application “Registration Form for Short-Term Visitors”
• B. Include your Curriculum Vitae with the application
• C. Sign a confidentiality agreement
• D. Submit a letter from the sponsoring institution (institution that employs the resident) stating the following:

• 	a) The sponsoring institution will be assuming the cost of the observer’s salary, fringe and malpractice insurance during the period the observer is at NYPH.
  	b) The observer’s current appointment status at the sponsoring institution
  	c) The dates of the observer’s rotation at NYPH.
  	d) The sponsoring institution’s department of Occupational Health Services has medically cleared the observer (including date of influenza shot).

• E. Observers coming from non-English speaking countries, a telephone interview to assess English proficiency is required.
• F. Pay the administrative processing fee of $500 in the form of certified check to Columbia University Medical Center – Division of Nephrology.

Note: A clinical observer is not allowed to have any patient care responsibilities or engage in any “hands-on” experience with patients and their families. Clinical observers must always be accompanied by their sponsor or a university/hospital employee while on patient/clinical care areas.
