For your convenience, laboratory requisitions will be sent electronically to the lab. Should you require a printed copy of your lab requisitions, please access your patient portal at myconnectnyc.org. Navigate to the menu, select “upcoming tests and procedures,” and locate the requisition entries to print them. Both laboratory tests and radiology studies will be in the same place.

For radiology studies, appointments must be scheduled by calling the Radiology Department at 212-326-8518. You can book your imaging services at any of the following locations:
